You have been selected to serve on a community outreach committee within your state’s nursing organization. The committee includes registered nurses of different specialties. At your first meeting, it becomes evident that not everyone is in agreement with a recent position statement about the role of spiritual care, with some members arguing they will no longer support the committee if the position statement is not revised or reversed.
As a nurse leader and change agent, how would you approach the committee? How could you draw from change theory to address these concerns and encourage collaboration on the committee?
Constant change is a reality within in the rapidly evolving healthcare system due to diversified populations and new technology (DeNisco, 2021). This creates several different implications that will influence these changes. The first is maintaining ethical standards. The nursing Code of Ethics was developed by the American Nurses Association and serves as guidelines for clinical practice (American Nurses Association, 2018).
These standards have withstood changes in nursing practice and healthcare but will constantly need to be reevaluated to ensure they are still applicable and maintain the highest standard of nursing care. The social standards of clinical practice must also progress to meet the changing needs of the diversifying population.

There are an increasing number of older individuals racial and ethnic groups that require specific and individualized approaches to healthcare (DeNisco, 2021). As changes are made to nursing practice, governing regulatory bodies such as State Boards of Nursing and Nurse Practice Acts (NPAs) will also need to be adjusted. With the growing roles of advanced practice nurses, NPAs may need to expand beyond regulating advanced practice registered nurses and include other advanced nursing practice roles as well.
One of the main focuses of healthcare is to improve patient outcomes while reducing the costs of care. Nurses have already been utilized to reduce costs and help meet provider shortages (DeNisco, 2021). This role will only continue to increase as advanced practice nursing roles continue to grow and become more defined. Any changes in healthcare are subject to political implications.
The nursing profession is becoming increasingly involved in advocacy for nursing practice. There are so many stakeholders in healthcare changes including patients, staff, politicians, state regulatory bodies, healthcare administrators and financial officers, and scientists and researchers.

The function of ethics in change management is collaborative. The project/program manager should understand people’s values, establish a secure place for people to express themselves, communicate truthfully, and be appreciative of comments received. Social ethics are the moral principles agreed upon by the members of a single society. Science and technology constantly interact with our social ethics.
Sometimes, new sciences and technologies clash with traditional values. At other times, science and technology encourage specific values like environmentalism and give them the strength to grow.
To lead change with integrity, you must ingrain ethics in your approach to change management. Communicate honestly and transparently the reason for the change. Treat employees as an essential part of the change process and ensure the company and its leaders care about the staff as people. Manage the change, assign clear expectations and responsibilities, encourage teamwork, and celebrate milestones.
Nurse managers must uphold standards of care while implementing change in the workplace. Remembering staff rights and conforming to policies and procedures. In order to implement change, organizations must establish procedures to ensure fairness and ensure that no harm is caused to those affected. Honor human rights and, lead with transparency, uphold the laws and Codes of Ethics.

Thank you for responding with this post. You mentioned that ethics in change management is collaborative. I can see that. It is equally important for leadership and employees to use ethical principles during the change process. Metwally et al., (2019) indicated that organizations in today’s society function in a highly competitive and changing environment that can push them to continuously adapt their organizational structures to such an environment.
Metwally et al., suggested that the success of the change process can be confronted by a barrier in the response of employees who will be impacted by change, especially when they lack readiness to change. Metwally et al., emphasized the importance of leadership and how it can shape the culture of an organization and help increase employees’ readiness to change by being ethical in serving as a guide, offering support, and being transparent (communicating).
Leadership can make a difference in the change process and ensuring employees’ adoption to change. According to Metwally et al., ethical leadership relates positively and directly to employees’ readiness to change.

Nursing leaders should realize change is constant and everyone may not adapt to change in the same manner. The role of ethics in management can go many ways. Managers or nurse leaders should understand that everyone have different values, and safe environment should be made for everyone to voice their opinion.
Communication should be open and honest, and no on should feel intimidated by the nurse leader. Despite the increasing inclusion of ethics and compliance issues in corporate training, the business world has common occurrences where the management lacks responsible conduct (Hauser, 2020). Many nurses feel that becoming involved in the political arena would conflict with their image and ethical principles, however, it is better to get involved and risk that association than to not push towards accomplishing professional goals, particularly because the policy formation that politicians control dictates the future of health care (James & Bennett, 2022).
A few years ago, a governor decided to stop the state’s hospital funding in my state. because of this change, many nurses lost their retirement plan once the hospital was bought out by a private sector. Choosing to be involved in politics can be crucial for nurses.

I agree! Many times professional nurses will experience situations in the workplace that challenge their personal and professional ethics. Morally courageous healthcare professionals, will however, find a way to make ethical decisions even if they are alone in their beliefs.
The nurse who is able to exercise this ability are able to strive to behave according to the nursing code of ethics, regardless of negative personal outcomes that may arise. These outcomes may include a tainted reputation, personal embarrassment, peer rejection, employer or peer backlash, and career ramifications.
Moral practitioners help to create a safe, non-judgmental caregiving environment. When unexpected danger occurs, nurses will take action to protect patients and themselves. They also will express empathy through words and actions, while forming strong connections with their patients.
The relationships that are created between nurse and patient foster a dialogue that helps healthcare professionals find personalized and effective care solutions. Nurses need to practice professional humility and flexible thinking in their practices. The nursing code of ethics encourages healthcare professionals to incorporate these moral guidelines into their personal lives as well.
According to the American Nurses Association (ANA), nursing ethics cover several common areas which include, end-of-life issues, bioethics, advocacy, caregiving and, moral courage, moral distress, and moral resilience.

A good example is in the area of personalized medicine, which has growing clinical applications throughout the healthcare system (Brothers & Rothstein, 2015). While genome sequencing opens the door to more understanding of each individual’s genetic makeup, and targeted therapies for illnesses, there are many ethical, social, legal, economic and political implications to this field.
There is a lot of sensitive health information created by genetic testing, such as predisposition to certain diseases or decreased responsiveness to standard medications, which could lead to genetic discrimination in hiring in employment or in access to life insurance due to their increased morbidity and mortality risk (Brothers & Rothstein, 2015).
This field may also increase disparity in healthcare due to the increased expense of testing, or the increased cost of medications targeted for a particular illness that is not the standard, insurance covered treatment (Brothers & Rothstein, 2015). If genetic testing becomes a regular practice it will affect primary physician practices as it will likely be used for health screening.
When practice changes happen in this area, the nurse leader will have to address the ethical implications with her staff, as there will likely be a broad range of opinions and values expressed. Then there will be ethical, social and economic implications to consider for the patients. The nurse leader may also need to become active in the political arena because new laws will need to be passed in order to protect the sensitive healthcare information that will be created.

Nurse leaders are change agents expected to use evidence-based practice skills and knowledge to address challenges that they may encounter while working in inter-professional collaboration settings. A core component of these skills is increasing understanding of stakeholders about critical issues and diverse perspectives to deliver quality care and attain positive patient outcomes.
Spirituality is a critical issue and one that requires in-depth appreciation among members (Nilsen et al., 2020). Having a certain stand that may be against values and perspectives among others requires a leader to model an acceptable approach to the matter.
Objectivity requires the leader to listen to all views and encourage members to be aware of the important role that spirituality plays in current healthcare environment and provision (Rizan & Das, 2018). Spirituality is increasing due to increased diversity and need to offer culturally-sensitive and patient-centered care.
As a leader, I would focus on attaining a consensus among members on the importance of spirituality and the need to review where it is not appropriate as a policy statement. I would implore all stakeholders to present their views on the subject matter and help them understand the need for increased prominence of spirituality. Spirituality allows patients to integrate cultural aspects into their care plans in collaboration with providers to enhance outcomes and better experiences (Timmins & Caldeira, 2019).
The ADKAR change model would be critical in helping the members appreciate the role of spirituality in healthcare. The model advocates awareness, desire, knowledge among members, ability and reinforcement. This implies that I will leverage the model to teach about the role of spirituality and develop an effective understanding of the issue (Wong et al., 2019). It will entail giving members awareness and developing a desire in them to implement these changes to suit the increased importance of spirituality. The acquisition of knowledge about the role of spirituality will allow them to make informed choices and arrive at a better approach.
