Evidence-based practice (EBP) is the careful and well-advised application of contemporary best evidence in concurrence with clinical knowledge and patient ethics. The first step in the Evidence-Based Practice (EBP) approach is identifying a clinical problem or concern. Even though it appears simple, there must be a solution to cope with this issue or collection of issues. The PICOT method is useful for providing guidance while coming up with research topics (DeNisco & Barker, 2016). The letters “P,” “I,” “C,” “O,” and “T” stand for target population, intervention or variable, comparison of intervention or variable with and without intervention or variable, outcome, and time required to achieve the outcome.
The following step in EBP is research. After creating a PICOT, look for any relevant historical and current evidence. There are also research steps included. You must have inclusion and exclusion criteria, which are frequently based on the ages of the participants in the study. The next step in EBP is research. After forming a PICOT, you want to search for all existing and current evidence related to your topic. There are steps related to research as well. You must have inclusion and exclusion criteria; this is often related to age of study. You must select appropriate databases to search, and you must critically analyze the evidence types. Not all evidence is created equally, there is a major
difference between evidence from a systemic review or meta-analysis with randomized trials versus observational descriptive or qualitative studies. According to DeNisco and Barker, the AACN has created a new evidence leveling system that essentially grades the evidence (2016, p. 476). Critically analyzing the newfound evidence is important to determine if the research is valid, of quality and applicable to your EBP. There are a number of models, checklist, and grading systems available to help with the critical analysis and organization of evidence (DeNisco & Barker, 2016). There are a number of databases that can be used to research, the GCU nursing library page lists several nursing databases that are great for the initial stages of EBP and research (GCU Library, n.d.). To name a few: CINAHL Complete is a good data based to start because it has quite a few specialties and is very nursing specific. PubMed is another one that has a gigantic database and is free to use. And COCHRAN library is a good one because it has seven different databases that related to providing medical providers with high-quality and independent evidence that can help with decision making in care.

I agree with you that when developing an EBP research topic, it is critical for the nurse to conduct a PICOT analysis, which will be critical in determining the feasibility of the research problem. The nurse requires adequate information about the research topic in order to understand the larger scope of the area of study and to be able to build on the already existing knowledge on a given research topic. These checks must be performed prior to carrying out the EBP so that the research work produced has a practical application in promoting quality care among the target population (Li et al., 2019). Some of the pertinent questions considered in the PICOT include: Will the study’s findings benefit the target population? Will the discovery be applicable in practice? Will the discovery support an existing concept or practice, or will it lead to the development of a new theory? Will the findings support the current nurse theory or provide evidence for a change in the current nurse theory? These are the key areas that the EBP research problem should address; otherwise, the problem must be re-examined and adjusted in order to have clinical relevance and importance in nurse practice (Duff et al., 2020).

Loosing your soul in nursing is not a difficult task and happens silently, overtime. Then all of a sudden something triggers your self-awareness that something is missing, your soul. In nursing its is no easy task to lose yourself and takes a concerted effort to not give to much of yourself. I have been burnt out more then once in my career. I have spent over 2 decades working prehospital, emergency room, and ICU. In this time period I lost my soul a couple of times. I have reinvigorated my flame for nursing. After very difficult personal experiences and evaluating what is important to me I now longer work in the high intensity environment. I have changed environments and work as a charge nurse in a specialty clinic. I am able to continue to have a positive impact on patient care minus the tons of stress. Not to say there is no stress but a lower intensity and a lot less. Change can be good but is a stressor all int’s own. One of the many positive things about our profession is the ability to change environments. This flexibility allows us to preserve or recapture our souls when they try and fly away.
