I define accountability as taking ownership of your actions and words. In my opinion, accountability is the most important trait to possess. Whether the actions/words ended positively or negatively, you must assume responsibility and ownership. I view responsibility as a similar term to accountability, but there are some differences. I believe responsibility is the process of taking ownership of and completing a role or duty, whereas accountability is taking ownership of the actions you performed. I define risk as exposure to danger or negative outcomes.
The APRN position description for Sanford Health touches on accountability but could go into further detail. The roles and duties are outlined, but I believe language could be altered to include the need of taking ownership of your actions. Obviously, the goal is to provide high-quality care at all times and to avoid mistakes, but when mistakes are made, it is crucial to have a provider who is accountable for their actions.
After graduation, I plan to embrace the concepts of accountability, responsibility, and risk by utilizing critical thinking skills. Before making decisions, it is crucial to weigh out benefits and risks. I personally try to avoid risky situations when able, but in certain circumstances risks must be made. In these situations, it is important to utilize critical thinking to assure the benefit is worth the risk.

When dealing with risks, I will take responsibility for the actions that I will perform. Additionally, I will take accountability for the actions that I have completed, whether the outcome was positive or negative.
As a hiring manager, I appreciate the question and find that these three traits are essential considerations when looking for a new employee. I define accountability as taking ownership of your errors (perceived or actual) and mistakes. Responsibility is a duty or commitment to a task or a role. Lastly, the risk is a danger that we seek to mitigate as healthcare providers. At my site, no language highlights these traits in the APRN postings.

At Mayo Clinic, they practice a value system called RICH TIES (Respect, Integrity, Compassion, Healing, Teamwork, Innovation, Excellence, Stewardship) (Brown Jr & Trumen, 2022). The values set an example of traits that all Mayo Employees should reflect. I believe that the organization should use these values and RICH TIES statements in all job posting to set expectations on the desired traits of an APRN at Mayo Clinic.
I plan to continue to reflect these values in my practice by keeping in mind that I need to be responsible for providing the best care for my patients and staff by taking accountability and minimizing risk through providing resources and support.

The common understanding of accountability is one in which a governing body (e.g., government, regional health authority, healthcare board, professional association) is in a position to mandate providers or organizations to meet certain goals or objectives. Denis (2014) posits that a culture of accountability in healthcare improves doctor-patient trust, reduces the misuse of resources, and helps organizations provide better quality care. With accountability comes responsibility.

Responsibility is the obligation to perform duties, tasks, or roles using sound professional judgment and being answerable for the decisions made. On the other hand, risk refers to the chance or likelihood that something will harm or otherwise affect a patient’s health or a healthcare organization’s effectiveness.
Advanced registered nurses are mandated to demonstrate accountability in all their professional dealings. Thus, implementing mandatory policies for accountability would change advanced registered nurses’ language and requirements. According to Davis (2017), they need to be accountable for themselves and others’ actions as stipulated by the American Nursing Association’s code of ethics.

They need to meet the growing needs and demands of patients and the rapidly evolving healthcare sector. They are also professionally accountable for expanding their clinical skill set and implementing evidence-based solutions.
After graduation and assuming my nurse leadership roles, I will highly consider the concepts of accountability, risks, and responsibility. As Little et al. (2018) state, I would assume the responsibility and become accountable for excellent day-to-day operations. With the same mentality, I will ensure that everyone assumes their roles and works toward achieving the common healthcare goal. Also, having a risk assessment model in place is critical in outlining any gaps and potential problems and opportunities available for my healthcare organization. As a visionary nurse leader, I will manage the three issues to strengthen positive outcomes and ensure that all stakeholders are satisfied.

“There can be no accountability without ownership” (Weberg et al., 2019, p. 636). I understand accountability to mean that one is responsible for meeting their commitments and performing their duties to the professional level of expectation. In other words, that one is reliable and dependable. I believe that ownership takes the concept of accountability to the next level.
Ownership is when someone makes a personal commitment to perform their duties reliably without fail and they take a personal stake in the outcome. Responsibility means meeting expectations that have been set before you. The concept of risk can be interpreted in various ways. There is acceptable risk and unacceptable risk. Acceptable risk occurs when the potential benefits of a treatment outweigh the potential risks. Unacceptable risk refers to unnecessary risks or situations where the risks of a treatment plan outweigh the benefits.
A current job posting for a Neonatal Nurse Practitioner at my facility states, ” NP’s are expected to act as an expert resource person for health care providers including nurses, residents and other ancillary staff”. This statement states an expectation, but I feel that it could be worded more professionally to elicit the concept of accountability. It could say, “NP’s are accountable as a resource for other providers and staff”.
In my future role as an advanced registered nurse, I will continue to practice accountability and responsibility as I do now to my peers, patients and institution. I will continue to bring all of my skills, knowledge and passion to improve patient care and support the healthcare system. I will only engage in risk that is of an acceptable level and when all participants are fully informed of the risk.

Responsibility is the ability to perform and complete the task and function of the role. To demonstrate effective responsibility, one needs to focus on the function of work and do it well. On the other hand, accountability is recognizing and acknowledging our responsibilities and being answerable for the outcomes of our actions, decisions, and mistakes. Accountability assumes ownership of the profession’s task, role, and characteristics (Weberg et al., 2019). Risk management is an essential leadership skill that involves identifying and assessing risk and coming up with a strategy to reduce their likelihood or impact (Conserve, 2022).
Upon review of the position description of an advanced registered nurse in our organization, the main content of the job description enumerates the responsibilities and task that comes with role. The word accountable is also included in a few lines describing customer service, and regulatory requirements. However, I think the statement that incorporates the concept of accountability into the job description is “supporting the company’s mission, vision, and core values and adherence to the compliance program and regulatory and policy requirements”.
As a nurse, I have to take ownership of the profession and acknowledge that I have an inherent obligation to make contributions to my profession that will be mutually beneficial to the entire nursing professional community. As a nurse leader, I need to promote accountability through clear definition of roles, encouraging team members to commit to specific outcomes, and outlining consequences if outcomes are not achieved. I also need to know how to manage risk as part of accountability in decision-making. As such, I need to learn how to work well under pressure, develop excellent communication skills, build relationships, learn to think strategically, and be knowledgeable of the regulatory and compliance standards.

As an advanced practice nurse one of my strategies to implement and increase participation in healthcare policy making is to join the ranks of my union 1199 SEIU to help draft policies that address the importance of universal health care or single payer option. For decades I have been helping my organization fight and lobby Congress to implement universal healthcare for all. During the COVID-19 pandemic we implemented a similar system in order to ensure that patients who had COVID-19 were not charged for treatment nor were they charged for vaccines this was a step in the right direction but we need to be more aggressive and ensuring that this nation which is the wealthiest nation in the world and one of the most advanced offers its citizens universal health care. Being an advocate for our patients it’s not just caring for them but fighting for them outside the hospital or health care setting. Although we have Obamacare we are still short of providing the most basic fundamental right to our citizens which is the right to health care. Health care is a right not a privilege and studies have shown that when people have access to universal health care they have better health outcomes. “Involvement in health policy is the most important role an NP can have in impacting positive change in health care that will benefit multiple generations of patients across institutions, states, the nation, and even globally; Together we can significantly improve the health of patients through our involvement on the front lines of health policy” (Chilton, 2015)

Health care policy affects every nurse’s daily practice, and determines who gets what type of health services, when, how, from whom and at what cost, therefore, the study of health policy is very important in professional nursing (Perez et al., 2020).
As advanced practice nurses, some strategies that can be implemented to increase participation in health care policymaking includes knowing about current issues, providing testimonies, writing letters to publication editors, participate in community and professional activities, build coalitions, contact policymakers, conduct research to create evidence, shape policy with evidence, lobby key individuals, join influential groups or organizations, identify policy problems and propose solutions, publish articles, serve on board, and volunteer on campaigns (Perez et al., 2020).
Joining a professional organization that is actively involved in policy advocacy that I am interested in and whose position statement is in alignment with my values and beliefs would be a very good start. I have participated in a number of policy advocacy campaigns before with issues that directly affected the health care workers and patients in my specialty. We got to speak with the lawmakers to state and justify our position statements. We also did some lobbying and several campaigns in different cities. We also submitted self-recorded videos of testimonies and encouraged our patients to also submit their own testimonies. We wrote letters to our representatives declaring our position statements on health care issues. As an advanced practice nurse and clinical leader, I want to continue doing the same, and be one of the people in the forefront of these advocacy activities.
